Dear Parents/Guardians,

The fifth grade is planning an extended-day field trip to Chattanooga, Tennessee, on Friday May 09, 2012.  We are so excited about being able to offer this fantastic learning opportunity to our students. 

            We will be traveling via luxury motor coaches from the American Coach Company. We will be accompanied by a representative from The Travel Store. The buses will depart from Nebo Elementary at approximately 8:00 A.M. on the morning of the 9th and will be returning at 5:30 P.M. the same day.  Upon arrival in Chattanooga, we will be visiting the Tennessee Aquarium for a tour of the River Journey and the Ocean Journey.  We will then be boarding the Tennessee River Queen for a narrated cruise and lunch. 

            The total cost of the trip per student will be $55.00. The balance in full is required by April 1^st.  While no student will be denied the opportunity to go on the trip, and contributions are voluntary, if enough money is not received to cover the cost of our trip by April 1^st, we will have to cancel. You may pay in installments of 10.00 per month; with the first payment due on November 1, or you can pay the balance in full.

Space is available for two chaperones per class.  Cost of adult chaperones will be $60.00 per person, due by May 1^st.  Chaperones will be selected from each classroom using the “lottery” method.  If you are interested in accompanying your child on this trip please indicate on the form below.  Please do not send in payment until you are selected.

            We believe our students at Nebo Elementary School are the best in Paulding County.  Our students will be representing our school, county, and our state on this trip to Tennessee.  As such, we are requesting that our students adhere to the rules of our classrooms, our school, and our district while on this trip.

            Thank you for your continued support.  Please complete and sign the permission slip below and return to your child’s teacher. Without a signed permission form, students will not be allowed to attend the field trip.
